  • Your Pet Data allows you to upload ancillary information such as your medical & legal documents including Pet Sitter Agreement, Care of Pet Directive, Pet Custody Agreement, & more.

  • Your Pet Data displays all of your pet’s essential information to include license & registration information.

  • These features provide the pet-parent with an organized way to keep track of all the essential data that may be needed quickly.

     

    Add Pepper’s Agreement, Legal, & Medical Documents

      Help ensure that your wishes are met and that all parties concerned with the well-being of your pet have the needed history and information by uploading the agreement / legal / healthcare forms below.

      Select the radio button at the LEFT of the form you wish to upload, then use the Choose File button to locate and select your PDF document file.

      Once uploaded, the file name will be hyper-linked for viewing at this location and you and people you wish to view the pet’s documents will be able to do so by logging in to the Vet-Caregiver Access page using the pet’s Pet ID. The Vet-Caregiver page is accessible from this website’s Home page.

      Press UPLOAD. File uploads are limited to PDF file type, at a maximum file size of 800KB.

      • To remove item, select radio button above and press REMOVE.

        Upload Additional / Misc. Form Files

        Add up to 7 PDF files for personal and/or first responder viewing. Follow these steps:
        1) Press “ADD UPLOAD FILE” to add a form.
        2) Name the file, Check the box to display the file to first responders, Add an optional note, and Press SUBMIT.
        3) Press the EDIT button to return to the form.
        4) Press the SELECT FORM FILE to choose a file.
        5) Press the UPLOAD button to add the PDF file.
        NOTE: DELETE removes the form and file and cannot be undone.
